When returning to school after an absence, parents/guardians must provide a signed & dated note stating the reason for the absence.
This can be turned in:
- IN PERSON by going to the correct Grade Level Office Assistant in the Counseling Center
To expedite the process and ensure accurate recordkeeping, please include the following information on all notes:
- Date
- Student’s full name
- Student’s Date of Birth
- Student’s ID number
- Student’s Grade
- Reason for absence
- Date(s) of absence and specific period(s) missed
- Parent / Guardian’s name (printed and signature)
- Parent contact number

Absences that are not cleared within 1 week will be marked as truant.
