We encourage students to take routine medications at home if possible but understand that sometimes medications or treatments need to be taken during school hours. In this circumstance we require the following:
- A "Request for Medication to be Taken During School Hours" form completed and signed by both the physician and the parent; and
- Medication to be supplied in the original labelled container.
Students with diabetes, asthma, and/or severe allergies may carry their emergency medications with them to be used when needed. A "Request for Self-Administration of Medication During School Hours" form will need to be completed by the physician, parent and student and turned into the health office. Medications carried by the student should have their name clearly written on it.
